Long-term care providers generally function as the primary care provider for people who live in nursing homes. These individuals are unable to live at home and require dedicated care. RHP professionals are there to assist with hospice and palliative care as well as managing ongoing health conditions.

Special challenges include:

Optimally managing hospital level problems with resources available in a nursing home such as IV antibiotics, X-rays, and oxygen therapy.
Deciding when and if a patient may need to go to the hospital based on their personalized goals of care.
Managing behaviors related to dementia and psychiatric illness in the elderly.
Preventative services such as offering appropriate vaccinations
Day-to-day management of chronic health problems such as heart failure, COPD, and diabetes.

We recognize that the transition to long-term and end-of-life care can be difficult for patients and their families. Our providers are always available by phone and are typically at the facility Monday through Friday during normal business hours.
