SNFist
A SNFist (pronounced “sniffest”) is a physician or mid-level provider who specializes in Skilled Nursing Facilities (SNF) care. Skilled Nursing Facilities provide care for patients who are not well enough to return home and require specialized care but are not considered ill enough to remain in the hospital.
SNFist care has grown to encompass all aspects of nursing home care adding long term nursing home care patients to the specialty. The combination is often termed post-acute/long term care and is abbreviated as PALTC.

RHP providers function as the primary care providers for many patients who live in nursing homes. Most of these patients are elderly with many health problems and very poor function. A dedicated physician is positioned to manage these patients optimally, with the assistance of one or more nurse practitioners / physican assistants.
When someone becomes unable to care for themselves requiring 24-hour assistance or “total care” (meaning assistance with all activities of daily living), they rarely have the resources to stay at home. Knowing there is someone they can trust to manage their medical problems in a nursing home can create some peace of mind during this difficult time.
