Local SNFist group is hiring an NP or PA to provide vacation coverage in the Post-Acute/Long Term Care (PA/LTC) setting. This position is PRN managing typical medical problems to assist patients in transitioning from the hospital to home in Chattanooga, TN; Rossville, GA; and Fort Oglethorpe, GA. Provider has flexibility in the days they are able to cover. No nights or weekends required. Experience in Post-Acute/Long Term Care (PA/LTC) is preferred, but other experience is welcome. Must have an active Georgia state license to apply. A Tennessee state license is also required and will be reimbursed if needed upon hiring.

We are seeking a Physician Assistant or Nurse Practitioner to join our team caring for patients in a skilled nursing and long-term care facility in Greenville, SC.

Three positions available: two 7 on / 7 off positions with 12-hour shifts and one Mon.-Fri., 8am-5pm position.

Call coverage is provided by a national telehealth group, with the NP/PA available for back-up and oversight of any hospital transfers.

Experience in post-acute or long-term care (PA/LTC) is preferred, but providers with other relevant experience are encouraged to apply.

Key Responsibilities

  • Serve as the primary care provider for patients in skilled nursing and long-term care settings, ensuring high-quality, patient-centered care.
  • Perform comprehensive patient assessments and evaluations to develop individualized treatment plans aligned with each patient’s goals.
  • Diagnose and manage acute and chronic conditions, coordinating timely interventions to promote recovery and prevent unnecessary hospitalizations.
  • Accurately document all patient encounters and care plans in medical records, including appropriate ICD-10 coding, to support continuity of care and compliance.
  • Partner closely with nursing staff, facility teams, and other healthcare professionals to deliver coordinated, compassionate care.
  • Educate patients and families about health conditions, treatment options, and preventive care, fostering understanding and shared decision-making.
  • Complete required Medicare recertification visits and documentation to ensure uninterrupted patient services and regulatory compliance.
